The CDPATH Process

Determine your patient's eligibility by checking the following criteria:

  • 18 years or older
  • Diagnosed with Crohn’s disease within the last 10 years
  • Have not experienced any serious disease-related complications
  • Either on a commercial healthcare plan or are uninsured

Order CDPATH test kits using the form below

CDPATH test kits are free of charge and include all required forms and shipping materials.

Complete a CDPATH Test Requisition form; give all test materials and paperwork to your patient

Help your eligible patient find a participating location for the required blood draw

Use the locator tool to find participating locations. The required blood sample can be drawn at a non-participating location (e.g., healthcare provider office) but then only the cost of sample shipping and test processing would be covered. (Phlebotomy charges would not be covered by Takeda.) The phlebotomist will send the sample and paperwork to the lab for processing.

Discuss results with your patient and encourage their participation in disease management conversations

CDPATH results are sent to your office approximately 7 business days from the date the blood sample is received by the processing laboratory.

*Serious Crohn’s disease complications are defined as bowel strictures, internal penetrating disease, or non-perianal surgery (bowel resection or stricturoplasty).
